Here's one that I noticed on my daily commute on I-405 through West
Los Angeles:
Just before Exit 55A (CA-2/Santa Monica Blvd) is:
Santa Monica Airport 3rd Right
Then, after Exit 55A but before Exit 54 (Olympic Blvd) is:
Santa Monica Airport 2nd Exit
So, the hapless driver goes past Exit 54 and finds that Exit 53 is the
interchange for I-10, which has options to go either East or West. No
posted signs give any guidance as to which option to choose.
Since I haven't seen anyone stuck in the gore point between East and
West, I guess the driver figures that since the Control City for I-10
East is Los Angeles, and the Control City for I-10 West is Santa
Monica, the route to Santa Monica Airport is via I-10 West.
The guide signs for another local landmark, the Bergamot Station art
gallery and cultural center, avoids this whole problem with a far
clearer sign:
Bergamot Station Use shield(I-10) West

Yeah, Oklahoma has this annoying affinity for "first right," "second
right," "third right," etc. Kansas has a much more sensible practice
of saying e.g. "KCI Aiport/Exit 14C". Although that wouldn't help much
in the California situation unless the ramps were posted as 53A and B.

That would not help here because, in accordance with Caltrans Exit
Numbering policies, "Downstream Exits" are not assigned separate exit
numbers.
In CA an exit subject to Exit Numbering is defined as "a divergence
from the mainline of the numbered route", which means that Exit 53 is
the point where traffic bound for I-10 diverges from the I-405 South
mainline; the downstream point where drivers must choose between I-10
East and I-10 West is not to be assigned an Exit Number.

One of the ways to beat that is to plan your route before you start,
then you an list in your notes what works best for you (I favore the rt.
number, direction, location-likely-to-be-on-BGS notation my self.
Regarding exit numbering, it would be nice if there was some
standardization, but ...

I suspect the reason you've not seen anyone stuck there in puzzlement is
that SM Airport is a general aviation airport that is used mostly by locals,
unlike LAX, so practically everyone who goes there knows where it is.
But you're right about the lousy signage.

I took a look at the "2nd exit" sign today. I missed the "3rd right" sign.
However, as the exit is the I-10 interchange, and the choices are "Los
Angeles" (east) or "Santa Monica" (west), I don't think it takes a rocket
scientist to figure out that the west fork is the correct one - but I agree
that the signage is poor, and not easily visible from the center lanes being
on top of the noise wall by lane 6 and only 4 feet wide.
