I have a friend with a 91 Accord with 285000 miles. 4 cylinder, a/t. It is
-7.0 degrees and a windchill of -20+. The car is fine in town, but on the
highway the temp gauge drops to C and the heater quits working. The
covered part of the radiator, and it gets "Less" Cold. The antifreeze is
strong, and full. The only thing I can think of is HEAD gaskets, etc?
ANY help would be appreciated.

How old is the thermostat?
If more than a few years, consider replacing with a new OEM
( = genuine Honda) one. Part costs no more than about $20.
I wouldn't think "failed head gasket" without more symptoms
(e.g. oil in the cooling water and vice versa). Google and
many sites discuss them.

I would suspect the thermostat. Replace it with OEM only.

Sounds like the thermostat is not working properly.
