Friend has a 1988, B2000. We replaced the head gasket, put everything back
together, and now when the truck is at idle (at a light for example) it may
idle fine for about 20 seconds then suddenly dies. Runs fine otherwise
It didn't have this problem before we changed the gasket. We verified all
the wiring and vacuum hoses, replaced the O2 sensor (fouled with anti
freeze, hence the gasket change). Any guesses as to what we should check
next.

Check the wiring to the idle solenoid in the carb. I had the identical
problem last year after overhauling my engine. The idle solenoid cuts off
the idle circuit to prevent the engine from dieseling. Mine had a bad
ground. If in doubt make up jumper wires and energize the solenoid
independent of the engine wiring harness. If it runs fine with the jumper
wires you've found your problem.
