Loose nut behind the wheel.

No, not locked out. The synchro ring system will do its best but shoving it
into 1st at highway speed will be near impossible. The trans will certainly
object to this abuse.

So ... what broke?

I'd say it IS in neutral. Maybe the select cables have broken (never seen it
myself but not impossible): look at the two cables on two levers on the
front top of the transmission. Moving the gear stick should move both shift
levers. If no movement, fix the cables. I don't think this is the case, as a
broken cable would make the movement of the gear stick "odd". Its possible
that the cables are moving the levers, but the shift shaft that goes into
the trans is not. Look for a roll pin or similar that secures the levers to
the shaft.

The shift levers move the internal selector rail mechanism. This may be
damaged inside the transmission, though likely not badly. Depending on the
age the car, you may find that getting another trans from a wrecker is
cheaper than getting yours fixed.

The engine vibration is a worry: you may have damaged the clutch system. Is
it possible that the vibration was there already, and you now notice it?

Bottom line is that if the cables and shift levers are OK, the trans has to
come off for inspection of it and the clutch.
