I have a 94 Altima that occasionally will seem to go into neutral when at a
stop light.
When I press on the gas, the revs go up, and then the car bangs back into
drive.
This only happens occasionally.
What could this be?
Fluid is in good shape and up to the proper level.

Low on transmission fluid or time for a rebuild. You've had the trans
fluid changed a couple times by now, right?

a fluid and filter change may actually help, small particles may be clogging
the filter, hurting performance

normal failure for this transmission is in the input clutch drum it is small
and way undersized for the transmission...get it rebuilt .do not service it
since fresh fluid may cause further problems due to the high cleeansing
properties of transmission fluid...sooner you get it done the better you
will be since most shop charge extra for hard parts the longer you wait the
more it will cost you to get fixed...ASE certified transmission tech...dont
buy a new computer for your car repair yours ask me for info

My '94 Altima has done this since new when stopped,  especially on slight
uphills.  I assumed it was the torque converter stalling (?) due to low
rpms.  Over the last 108,000 miles it has come and gone seemingly - but not
necessarily - with how the Nissan dealer sets the idle when it goes in for
routine service.

You are losing pressure to varous servos and bands in the transmission.
This can happen for a number of reasons including, but not limited to:
Low fluid level
Aerated fluid (normally caused from overfilling)
Clogged filter
Malfunctioning pressure regulator
Clogged valve body
