Help emissions failed

I just bought a 87 nissan maxima  and it failed on the smog test ie:
high CO levels and HC levels
15 mph    4.73%   and  330 ppm
25 mph    4.63%    and  339 ppm
miles on car 239,000
what could make these amount so high
visual inspection passed

Yes, you're sure running rich all right. There can be many things
causing your problem including... but not limited to:
Engine not warming up properly
Coolant temperature sensor malfunction
Throttle position sensor potentiometer noisey
Clogged air filter
O2 sensor
