I have a 2004 Quest with 75,000 kms. Twice in the last couple of
months I have experienced what I can only describe as losing 1st gear.
It happens on start up. Turn the van on and when put in drive it feels
like you aren't going anywhere when stepping on the gas. Turn the van
off and back on again, problem solved.
The dealer is suggesting replacing the tranny fluid. I asked if old
fluid would manifest itself like that; he didn't answer. So I have to
flush and replace the fluid first, then if it happens again, they will
look into more.
Has anyone had this issue or any suggestions?

Sounds like a line pressure issue. Changing the fluid *might* help, but
probably not.

This is a common complaint,
There is a TSB on this problem , the TCM (transmission control module) needs
replacing.
