Does anyone know if you have to remove the exhaust manifold in order to
remove and replace the starter

Boomer,

You don't HAVE to, but your knuckles will thank you so very much if
you do! I've done it both ways on 85~90 98s. Pull the fan(s) out
first, then the manifold, and be careful not to damage the radiator
when you remove the manifold (normally the fans protect the inside
side of the radiator from damage by slipped tools and such).
