Usually on stolen vehicles.
Seriously, your vehicle has hidden numbers on the frame in various
places. You can contact your state auto theft investigators and they
will find the VIN on the frame. They can straighten everything out
faster than anyone. If the vehicle is stolen, and you  bought it from a
dealer, you will certainly end up with a better vehicle for free.

 Much more likely that there was a serious typo

The VIN number is the actual legal year model regardless of the paper
title. The 10th number is the year model the unit is.

Actually just went through this yesterday and today. Talked on phone
with the dot in our state here (WI). He had me go out and read the vin
off the dash/window area. Found the incorrect missing digit. Said to
mail data including title to attn to him and would take care of it. We
decided to go to DMV locally here today and went to window and brought
our vehicles original sales slip stating proper vin and title with
incorrect vin on it and the person made copy of it and typed it in and
printed new title for us right then and there. Now just gotta renew
license plates for the critter.

The problem has happened to me twice.  In the first instance, the VIN
had one error, either transposed numbers or one digit off.  The MVD
office verified all the VIN tags and accepted the correct number but
it took two license cycles to correct.  

In the second case, I got a title from Illinois and when the MVD
people made up our Arizona title, they left out a number.  I noticed
the error (I'm anal about that sort of stuff) and returned the next
day.  The offending clerk graciously redid the title.

Best to get it taken care of ASAP.

I ran into it one time while bringing a converted high flotation truck
across the border.  The border guard had my driver in his office & me
on the phone:

Border guard: "The VIN and the title don't match - is this vehicle
stolen?"

Me:  "Ah ---- no sir"

We ended up unloading the truck and leaving it in the customs compound
for a couple of weeks while the vendor sorted out the paperwork.  It
turned out to be a case of a number getting smudged on a fax and then
incorrectly entered from that.  I think it was a lot more alarming for
my driver than it was for me.
