I have a 1986 4x4 pick-up that is getting louder and louder in gears #1,2,3,
& 5 (4 is direct drive)!  How many more miles can I go before it dies?  What
should it cost for me to have it rebuilt?  Any other suggestions?  The truck
is an Xtra-Cab with AC that otherwise runs great. I really don't want a
newer truck.

Try draining the tans and refilling with synthetic fluid.  If you are where
it gets really cold go to a slightly higher viscosity.  In mean time look
for a low mileage used unit.  Rebuilding a trans --- just the parts -- is
terribly expensive and skill of some of the rebuilders is questionable.
