Interesting true story from my nurse friend:  One of her patients had
congestive heart failure a year ago. He's 84. He hasn't uttered a word in a
year. He's tube-fed and on a ventilator. He can't signal in any way. My
friend can only guess what it must cost to keep him alive for yet another
day. He gets "code blue" treatment - the staff must do anything they can to
keep him alive, because he doesn't have a DNR order and he never designated
a medical proxy.

His family could legally ask that the heroic measures stop. Guess why they
don't do this.
