Receipt etc when selling a car

Anyone know where I might find a std downloadable receipt etc, for selling a
car privately? Listing buyer, seller addresses, price, sold as is, or as
seen etc.
I know what needs to be included on the bill of sale, I'm thinking of a
nicely laid out template where one can just fill in all the details and
print it off whenever one sells a car.
Had a google for one but all I got was the what should appear on the
receipt. No sample layouts.
I've always made them up in the past, but I'm sure others have made a better
job of composing something than myself.

One thing I would add to that is the TIME of sale alongside the date - last
thing you want is your buyer zipping off in the new car past half a dozen
speed cameras - and then YOU try proving that they had already picked it up
when all you had written down was the date.
